Here are instructions for your visit:

Please expect that we will need to review your tests, procedures and treatments.  Contact our advocate so that we can obtain your pathology slides from any biopsy or surgical procedure, radiology reports, your medications and medical summary.

All our patients are getting appointment notifications via email and phone.

If you need to cancel or re-schedule your visit, contact us as soon as possible.

We suggest you bring only two visitors to office appointments.

Please bring your ID and insurance information.

Contact your primary care physician for a referral if needed ahead of time.

Contact your insurance company to assure that you do not need approval ahead of time.

Make sure you know the address since we have multiple locations.

Prepare the questions that you want to ask by writing them down ahead of time.

Most patients are very nervous at the time of visit and forget to ask  questions.

We leave plenty of time for questions and discussion.
